Dental Assessment Forms
When the Health Assessment Form is completed, a separate Dental Screening Form must also be completed. This is a requirement per NC Child Care Rule 10A NCAC 09 .3005 Child Health Assessments. The child’s health assessment must include a dental screening, which may be recorded on this form. Click on the link above to print this form.
**The dental form may be completed by a Dentist, Dental Hygienist, Physician, Physician Assistant, Registered Nurse, or Other Health Professional.
Immunizations and Health Assessments
Families have 30 days from the first day of attendance to complete a health assessment and updated immunizations. Please use the link above to access the CMS Immunization and Health Assessment Page for more information and to print the required Health Assessment Form.
