References: Applicants must submit two supervisory reference forms, including one from their current supervisor.

1. These references must be dated within six months of the application submission. Please add two supervisors to the reference section of your online application. The application system will send an automated email to the email address(es) you provide.
2. Please ensure that you follow up with your references, as your application will be considered incomplete if you have "pending" references. You can monitor the status of your references by clicking the "References" tab within your application.

1. Current CMS Employees: You can obtain your CMS evaluation via MyTalent or by sending a request to PerformanceMgmtDept@cms.k12.nc.us. Ensure you provide your CMS employee ID and specify your role at the time of the evaluation.
2. Non-CMS and Out-of-State Candidates: Provide a copy of your 2025-2026 performance review. The document must clearly display your name, the name and signature (physical or electronic) of the evaluator, and the date of completion.
3. Alternative Documentation Requirements: If a summative evaluation for 2025-2026 is not available, you may submit the following (listed by preference):
   • A PDF letter from your supervisor detailing your 2025-2026 performance and justifying the absence of a formal review.
   • Your 2025-2026 summative evaluation accompanied by a supervisor's letter explaining why a 2025-2026 evaluation was not issued.
   • Note: Individual classroom observations, self-assessments, and PDP evaluations are not valid substitutes for a full summative evaluation.

1. In-State Licensed Applicants: Please obtain your Educator License through the NCDPI website. View detailed instructions.
2. Out-of-State Licensed Applicants: Provide the following documentation:
   1. Copies of your Administrative and State Teaching Licenses
   2. Official transcripts for both undergraduate and graduate degrees
   3. A copy of your Praxis Test Scores or any applicable test scores

   1. Provide unofficial transcripts indicating program completion or expected completion (within two months of your principal preparation program date); OR
   2. Provide a formal letter from the executive director of your program confirming the completion of a principal preparation program.
