School Nutrition Services • CMS

All requests for special nutritional needs must be submitted to School Nutrition Services on an official CMS Diet Order Form signed by a licensed medical doctor (MD) or recognized medical authority.

Forms must be submitted each time a student's diagnosis or treatment plan changes during the school year. Incomplete forms cannot be processed and will be returned to the parent/guardian for completion.

Upon receipt of a completed form, School Nutrition will modify menus and instruct cafeteria managers regarding the student's special diet. A registered dietitian supervises all special nutritional needs management. Parents are encouraged to review appropriate food choices with their child.

Families managing diabetes or food allergies without a formal diet order form can review nutritional info, carbohydrate counts, and major food allergens using the resources below.

Peanut Policy Note: CMS School Nutrition does not purchase or serve peanuts or peanut-containing products. A diet order form specifying only a peanut allergy is not required.
