School Nutrition Services • CMS
All requests for special nutritional needs must be submitted to School Nutrition Services on an official CMS Diet Order Form signed by a licensed medical doctor (MD) or recognized medical authority.
Forms must be submitted each time a student's diagnosis or treatment plan changes during the school year. Incomplete forms cannot be processed and will be returned to the parent/guardian for completion.
Upon receipt of a completed form, School Nutrition will modify menus and instruct cafeteria managers regarding the student's special diet. A registered dietitian supervises all special nutritional needs management. Parents are encouraged to review appropriate food choices with their child.
