Student Information Form to Complete

STUDENT INFORMATION FORM:

On Tuesday, August 15, 2023, the North Carolina legislature passed the Parents' Bill of Rights. To better serve our families, CMS has created a dedicated webpage that includes information, forms, & resources that will help parents/guardians access information, request information, report concerns, or submit objections, opt in for- and receive notification about specific things as it relates to their child(ren) who attend our schools.

All parents/guardians are requested to complete the 2023-24 Student Information Form by Monday, September 11. The form is for collecting information related to three policies and includes:

  1. Name and pronouns for your student
  2. Opt-in to reproductive health and safety education programs
  3. Opt-in to protected student information surveys
