Parents Bill of Rights - Student Information Form
![]() Greetings Whitewater Families! On August 15, 2023, the North Carolina legislature passed the Parents' Bill of Rights. To better serve our families, we have created this dedicated webpage that includes information, forms and resources that will help parents/guardians access information, request information, report concerns or submit objections, opt in for- and receive notification about specific things as it relates to their child(ren) who attend our schools.
Please complete the Student Information Form by clicking on the link below. This form is required to be completed annually for all CMS students. This year’s form combines the Student Information Form and Health Screening Opt-In Form that were separate last year.
