Student Religious Obligation Leave Request Form

If you wish to request that a religious leave for your child be approved, please download and complete this form. The form must be submitted to the front office at least five days prior to the first requested day of the absence. Submissions can be emailed to deena.lunan@cms.k12.nc.us
