ADDITIONAL DOCUMENTS REQUIRED BY 30TH DAY OF SCHOOL ENROLLMENT
(If your child is placed in the Pre-K Program)

  • Child's up-to-date Immunization Record
  • Child's most current health assessment (on NC form) - completed within 12 months prior to the first day of school.

Gather Required Enrollment Documents

CMS requires a parent or guardian to provide the following documents to apply:

  • Certified Copy of Birth Certificate (Birth Certificate Requirement Form)
  • Immunization Records
  • Proof of Residency - Please note: if you cannot produce a current utility bill, a home or apartment lease contract will suffice until you can provide a current utility bill (required before starting school).
  • Submit Additional Documentation/Qualification Criteria for Pre-K Students (required to enroll in the free program)

Please be sure to review the documents accepted for proof of residency and double-check that your child's immunization records are up to date and reflect all the requirements of the state of North Carolina, which may vary from other states/countries.

Next Steps

Once you have completed the online application form and uploaded the required documents, you will receive email notification of completion or a request for additional documentation. However, the process is not completed until CMS district staff review and process the documentation. CMS processes new student applications in the order they are received.

Once the application is processed, you will receive an invitation to schedule your virtual screening appointment. Parents will check their Family Dashboard for placement information.
