Hardship Caregiver

To enroll a student in Charlotte-Mecklenburg Schools (CMS), you must be one of the following:

• Parent
• Legal guardian
• DSS placement

If none of these apply, you must complete a Hardship Caregiver application.

Important Information:

• The Hardship Caregiver form is valid for one school year only.
• You must renew it each year if the student continues living with the caregiver.
• The form must be notarized by a North Carolina notary before submitting.
