Hearing Screening

Dear Parents, 

All Pre-k, Kindergarten and 1st grade students and students new to CMS will have a hearing screening performed by the school’s Speech-Language Pathologist unless the school is notified in writing by Monday, August 28th, 2023.  If a student does not pass the hearing screening after two attempts, the Speech-Language Pathologist will send a “Consent for Hearing Evaluation” permission form to the parent.  Please return this form to the SLP by the required date.  

Parents of students being monitored for a history of hearing loss should be aware that their child will also receive this consent form.  Without signed permission, students WILL NOT receive a free hearing evaluation by the school’s Audiologist.
