Novant Partnership - Free Immunization

Dear Parent or Guardian,

The Novant Health Community Care Cruiser is partnering with North Ridge Middle School | 7601 The Plaza, Charlotte, NC 28215 from 10:30am. to 3:00 p.m. providing childhood immunizations free of charge to 6^th, 7^th, and 8^th grade qualifying students on May 17th, 2024. To participate, please complete the forms sent home with student and return them by May 10th, 2024.

The Novant Health Community Care Cruiser provides free access to high-quality medical services that includes childhood immunizations (VFC Program). Children who qualify for the VFC program include those with no insurance, have Medicaid, or are Medicaid eligible ages birth to 18 years in the Charlotte area.  Please fill out forms that will be coming home throughout this week or let us know if you need a form. Please return filled out forms as soon as possible. If you have any questions, please contact the school and we will be happy to help provide any needed information.
