Afternoon Transportation Changes

This online form MUST be submitted by 3:00 pm on the date the transportation change is needed: bit.ly/prmafternoon or www.cmsk12.org/parkroadES and click on the Afternoon Transportation Changes link in the Quick Links section. Any submissions after 3:00 pm will not be honored and your child will be sent home via the usual mode of transportation. Submit this form for each child who needs the transportation change.

Do not call the school or send an email or text your child's teacher requesting an afternoon transportation change. Do not submit a change for an upcoming day as the submissions are sorted by date.
