Registration & Change of Address at QHMS

    What do I need to bring to the school to register my student?

     

    1. Two ​​Proofs of Residency
    2. Completed registration packet
    3. Sign the Student and Parent Acknowledgement and Pledge ​the Code of Student Conduct: ​
    4. Immunization Records​ 
    5. Completed school forms
    6. Birth Certificate
    7. Report Card/records from prior School Year if possible

    ​How do I update my address? 

    1. Complete a Change of Address Form
    2. ​Provide two Proofs of Res​idency

    • Tuesday By Appointment Only
    • Wednesday 9:00-3:00
    • Thursday 9:00-3:00
    • Friday By Appointment Only
